How to Enroll

Health Net continues to update available plans for County of Santa Clara employees. This includes the Health Net SELECT Point-of-Service (POS) health plan option. Please visit the Plan Details page to read about the plan benefits and coverage details that are right for you!

Some POS plan basics

When you enroll in a SELECT POS plan, you must choose a participating physician group (PPG) and then select a primary care physician (PCP) within that PPG for the Select 1 (HMO) tier. Only the Select 1 HMO PCP and PPG will be printed on your ID card. Follow your employer group's enrollment instructions and deadlines.

Each covered family member may choose a different PCP and PPG based on their individual needs for the Select 1 (HMO) tier. Members may also access OB/GYN services in accordance with plan guidelines.

A SELECT POS plan offers three benefit levels:

• Select 1 – HMO
• Select 2 – PPO
• Select 3 – Out-of-Network

You are required to select a PPG and PCP for the Select 1 (HMO) tier; however, you may choose to use your HMO (Select 1), PPO (Select 2), or Out-of-Network (Select 3) benefits at the time you receive care.
