Find a Provider – FAST!
With Find a Provider, you'll quickly find the most up-to-date listings of qualified in-network doctors, urgent care centers, hospitals, and other types of healthcare providers near your home or work.
For County of Santa Clara – Select POS
- Launch Find a Provider.
- Enter a location (street address, city, county, or state).
- Further narrow your search by Plan/Network and select one of the following:
- Select 1 (HMO): Choose POS – Small/Large Group (tier 1)
- Select 2 (PPO): Choose POS – Small/Large Group (tier 2)
- Select a type of provider (doctor, hospital, medical group, etc.) to get your results.
- Finally, you can choose Print Results to print your search results.
To enroll or change your primary care provider (PCP), you’ll need specific details about the doctor you choose.
For Select 1 (HMO), selecting a PCP is required. When you find a doctor, click Show Details in their profile to locate:
- Physician ID#
- Group/PPG ID#
You’ll need both numbers to complete your enrollment or make a change.
Important: Not all doctors can be selected as a PCP. Make sure the doctor is listed as a Primary Care Provider (PCP) by looking for the green check mark next to "Primary Care Provider" in the Show Details view.

You'll want to visit an urgent care center when your condition is serious but not life-threatening. With wait times in California emergency rooms averaging 164 minutes,1 urgent care centers (UCC) can save you time and money, and are staffed by physicians. They are also useful when you are out of your main service area.2 Use Find a Provider to find your nearest UCC.
